Our Focus
While we help people with their immediate needs, we focus particularly on the long term changes that Australia needs to make if we are to treat complex chronic conditions better. We work to improve services, access to information and life skills programs through action research. We advocate towards changes in funding, policy and research priorities that improve health and quality of life.
Since 2001 we have provided services for over 6000 Australians who live with:

and also for people who fall through the gaps of current Australian health, disability and community service programs and services. Much can be done to prevent unnecessary disability.
Australian Approach To Integrated Management and Partnership Care
The pivotal goal of Bridges & Pathways is improved daily care. While other agencies are focusing on the causes of these conditions and the biomedical approach, our purpose is to improve current services. To achieve our goals we work with the General Practice, Allied Health, Medicare Plus, Australian Better Health Initiatives and Chronic Care Service Reform Programs. [ Services 2 ]
Our findings confirm that the proposed tailored integrated management Team Care arrangements helps bridge the gaps and provide a pathway to move care from current best practice to evidence based care.
