1. Casual Driver/Pit Crew Registration (A.I.D.K.A. 2003.11)
This form is to be used whenever a Driver/pit crew without a Licence wishes to drive a kart or enter the pits. This Driver/pit crew is to be a member of an A.I.D.K.A. Club and is not to participate in any racing at all. The current fee for casual Drivers/pit crew as per Appendix “E”. The form and money must be forwarded To the A.I.D.K.A. Secretary within seven (7) days of the meeting.
2. Accident/Incident Information (A.I.D.K.A. 2003.15A)
This form is the standard form for reporting to A.I.D.K.A. the details of any accidents or incidents which may involve insurance. Please use this form whenever it is possible for an insurance claim. The form must be forwarded to the A.I.D.K.A. Secretary within seven (7) days of the accident/incident.
3. Accident or Illness Claim Form
Whenever a claim is made for insurance this form must be filled out and forwarded to the A.I.D.K.A. Secretary.
4. Attending Physicians Statement
This form must be filled out by the attending physician and forwarded to the A.I.D.K.A. Secretary.
5. Employers Statement
If the insurance claim involves a self employed person then this form is not required otherwise forward it along with the other insurance form to the A.I.D.K.A. Secretary.
6. Licence
Application (Form 1 2009 AIDKA_Licence_Application)
Licences for adult, officials and junior drivers along with officials will only be accepted on this form with no exceptions.
7. Appeal (A.I.D.K.A. 2003.16)
To any member who requests one, please contact your Club Secretary or Club Steward.

9. Forms 2, 3, 4 & 5 are necessary for an insurance claim to be processed promptly.
10. If self employed, form 5 is not necessary, but proof of income is required (e.g. tax returns, statements etc.).
